From time to time it might be necessary to refer a patient to a consultant for further care. In such cases we will do our utmost to ensure that referral letters are sent promptly from the practice and wherever possible, via Choose & Book.
Your responsibilities to us are
to ensure that you keep the practice and the hospital informed of any changes of address and/or name so that hospital mail will reach you at the correct address
to recognise that any delays in receiving an appointment are usually outside our control
to keep your hospital appointment or cancel it in good time so it can be given to somebody else on the waiting list
